By signing below, I acknowledge that I am 18 years of age or older (Under the age of 18 need parent or guardian signature below) and I agree to the following:
I acknowledge that exercising can be an inherently strenuous activity and that no event is without risk. I understand that classes & events held by Spark Workout Studio may be physically strenuous and I voluntarily participate in them with full knowledge that there is risk of personal injury, property loss or death. I have consulted with my physician regarding my physical capacity to participate in this event and am following my physician’s advice.
I hereby WAIVE AND RELEASE all claims, demands, causes of action of any kind against Spark Workout Studio, its owners, officers, employees and instructors for any injury, accidents, physical or psychological conditions I might suffer related to my participation in classes offered by Spark Workout Studio. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against Spark Workout Studio, its staff, or its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.
Our studio is a resource for healing, which requires clear and healthy boundaries, therefore please know that for your protection, teachers are prohibited from engaging in romantic or physical relationships with students.
